Complete and sign a Payment Order Form. Your form must include the following:
- 1. The name and physical address of the beneficiary customer.
- 2. The name and physical address of the beneficiary bank.
- 3. The SWIFT address/ABA# or FED Wire #/ Transit#/IBAN # of the beneficiary bank and intermediary bank (if necessary).
- 4. A reason for sending the wire.
- 5. The desired transfer amount and currency type.
